A long time ago, 6 hard-working humans split a maybe-faerie into 6 parts. And then, Albion tried to dig a hole to Avalon and the six were part of the collateral damage of that, getting buried in the place that would later be called the Spirit Tomb of Albion.

Later, Albion excavators found the parts and sold them to the then-current head of the El-Melloi family, who modded them into a trinity of Mystic Codes that produced an inexhaustible supply of magical energy. The Mystic Code trinity served as El-Melloi's Supreme Mystic Code. The Lord El-Melloi of old actually had the option to unite all 6 parts like that, but he was smart enough not to do that. The pro of fully restoring her would be generating even more energy, but the con would be that it would make her a complete being with a will of her own and that A) not a relevant thing for a Mystic Code to have, and B) begging for unfortunate consequences in the future.

Flash forward to November 1994. Kayneth brought the item to Fuyuki but lost her when the Hyatt Hotel got bombed. The item was retrieved by a mage couple who came to Fuyuki to loot during the Grail War, and to lay low, they built a workshop in Semina Apartments. Those two weren't as prudent as the OG Lord El-Melloi, so they spent the next 7 years fully restoring the maybe-faerie, and the end result was a blond child with elf ears, which we'll be calling Little Red Riding Hood.

In this state, the Little Red Riding Hood is a living Grail. She circulates amounts of magical energy comparable to the leyline within herself and limitlessly grants wishes. But also twists fate in a way that gradually alters people's mental states into wanting to make a wish to her, which is immensely messed up.

The couple who owned her was a pair of mage looters to begin with, so yeah, no way those guys were resisting the mental distortion. They began fighting nonstop over what to wish for, regularly asking Little Red Riding Hood for wishes that contradicted the last day's wish, cyclically driving her away out of fear then bringing her back out of addiction, and abusing her with magical tools. During the "drive her away" phases of the cycle, she wore a red hood to hide her hair and ears, and since she can't grant her own wishes (not that she had any), she sometimes requested the neighbor to press the elevator button for her. That's what Hollow's intro horror story is based on.

And that brings us to August 2003. Her body produced a bladed weapon resembling an enchanted sword and the couple killed each other fighting over it, The End. Although Little Red Riding Hood asked her neighbor for help once there was no one else left, A never opened the door, so the Little Red Riding Hood disappeared somewhere. She's autonomous, but she still can't keep her body operating without a mage doing regular maintenance, so unless anyone rescued her, she's dead for good.

Not in the FsF timeline though. Things here are different thanks to the Himuro no Tenchi background circumstances. Before the couple's murder, while Ayaka was doing her secret Grail War damage control operations, she cast some minor hypnosis on A asking him to, without risking his life or anything, reach out for the girl if she ever needs help. Due to that, on the night of the incident, A opened the door and let Little Red Riding Hood into his apartment instead of committing to ignoring as he did in the horror story.

But that didn't go where Ayaka expected it to go. A was always dedicated to being alone, which is probably why he didn't open the door in the Fsn timeline, but Ayaka's words never lever his heart and he followed them to the letter, so that's a thing that he had to rationalize as "it's because I fell in love with her". Intuiting those thoughts, the Little Red Riding Hood gradually transformed into someone identical to Ayaka, with her blond hair being the only thing left of her original appearance.

One month later (September 2003), the Little Red Riding Hood was dying from the lack of a mage's maintenance, while A's mind was getting transfigured by the Little Red Riding Hood's gimmick, and that led A to the conclusion that he could reconcile his love and his solitude if he and Ayaka fused with a single individual, so he proposed to give her all of himself. She granted his wish, fusing Little Red Riding Hood and A into our dear Fake Ayaka, a blond Ayakaface with both A's and LRRH's memories.
